Please read the following information before submitting this Performer Registration Form:
Performer Registration Form is DUE BY June 30, 2011. Please go online and type in http://after5et.blogspot.com in you address bar and then click on “click here to register” on the right side or email your full name and contact phone number to after5info@gmail.com or fill out this form and return it to any After 5 staff.
Important to note: Submission of this registration form does not guarantee a slot on the program. Although we would like all who sign up each to have the opportunity to perform, depending on the number of registrations we receive, it is sometimes difficult to accommodate everyone on our program. Therefore, the program committee will notify chosen applicants by July 2, 2011. We appreciate your support of this event, and would appreciate your flexibility and understanding in this matter.
Participants of 21 years of ages and over, women and men are invited to share their talents in comedy.
You are performing for FREE and competing to become 1 of the three (3) winners.
Our program consists of a total of 10 performers competing for 1st ($300), 2nd ($200) and 3rd ($100) place prizes.
Each performance slot is limited to 5 minutes in order to accommodate all 10 performers.
The winners will be selected by audience’s applause.
Every Participant in there performance will not use any content that would be deemed offensive or inappropriate for general public viewing. After 5 reserves the right to immediately terminate a performer from the stage and they will be eliminated from winning.
Every one is prohibited from bringing alcoholic beverages into the Guild or its premises.
All 10 performers MUST be at the show and logged in 30 minutes before show time.
